Office Policies

• I charge the full rate for appointments that are missed or that are canceled with less than 24 hours notice.
• All fees are due at the time that services are rendered and products are received.
• There is a $25 surcharge for returned checks.
• I protect your personal information, follow the federal regulations of the Health Insurance Portability and Accountability Act (HIPAA), and will not give out your financial or medical information to any other entity without your express written permission.
• I pledge to never discriminate against my patients due to age, disability, religion, national origin, political affiliation, race, ethnicity, gender identity, sexual orientation, or any other reason.
